Metro South Aged Care Assessment Team (ACAT) is looking for a Social Worker to join their team. ACAT is a multi-disciplinary team of allied health and nursing clinicians whose main function is to comprehensively assess the care needs of frail older people and assist them to gain access to the most appropriate types of care, including approval for Commonwealth subsidised aged care services.Job details Position status Fixed Term Temporary

In delivery of assessment services, the role operates within the Aged Care Act 1997 and its Principles, and aligns with the Commonwealth Department of Health guidelines and practice recommendations.

  • Comprehensive assessment of older people involving the client, family/carers, service providers and GP’s
  • Multidisciplinary team (Nursing, OT, PT, SW) with professional reporting to professional lead/s, peer supervision within the team and HP6 Operational Director
  • Previous experience with ACAT highly desirable.
  • Acute and/or rehab aged care experience highly desirable.
  • Community health aged care experience valued.
