Where you’ll be working
Prince of Wales Hospital and Community Health Service is a Level 6 Tertiary Referral Hospital with an inpatient bed base of 370. Each year we care for more than 70,000 patients in our Emergency Department and have around 50,000 admissions to the inpatient units. POWH offers an outpatient service and rural outreach service and provides more than 900,000 occasions of non-admitted patient care each year, including innovative virtual models of care. We have transitioned the majority of inpatient units and wards to the Acute Services Building (ASB) including a state of the art Emergency Department and Intensive Care Unit. The Clinical Nurse Specialist Grade 2 will triage referrals, perform comprehensive assessment, and participate in client case reviews. Raise awareness about dementia and the advantages of early intervention via direct client/carer contact, improve access to information and resources about Dementia for people with dementia and their carers, and provide flexible support and advice to ensure the ability of carers to continue their primary caring role and make informed choices about their future.What you’ll be doing

  • Relevant Post graduate qualifications in aged care or dementia care and at least 3 years recent experience working in the aged care, dementia or aged care rehabilitation clinical area, or such other qualifications or equivalent experience deemed appropriate.
  • Demonstrated ability to manage complex clinical care and service delivery issues and problem solve through the development and use of clinical management plans.
  • Demonstrated ability to exercise extended autonomy of decision making while working as part of a multidisciplinary team and apply professional and ethical boundaries when dealing with complex situations.
  • Demonstrated skills in the development & facilitation of patient and carer education and staff as required.
  • Demonstrated high level written and verbal communication, interpersonal skills and the ability to identify key messages, issues and concerns when communicating with others.
  • Detailed knowledge and understanding of harm minimisation and evidence of leadership in safety and quality innovation.
  • Demonstrated competence in the use of health-related information technology.
