Where you’ll be workingPrince of Wales Hospital is a 440 bed tertiary referral facility with specialties unique to this campus such as hyperbaric medicine, acute spinal cord injuries and interventional neuroradiology. Each year we support approximately 25,000 emergency presentations, 2,500 operations and thousands of outpatient treatments. We provide a 24 hour Emergency Department (ED) supported by an Intensive Care Unit and pioneering telehealth facilities in stroke and COVID-19, to connect the team in Randwick with specialists across NSW Health. SESLHD is committed to enabling our community to be healthy and well, and to providing the best possible compassionate care when people need it.The Community Dementia Clinical Nurse Consultant (CNC) is a Registered Nurse who has responsibility for providing expert clinical advice, coordination, and consultancy services, to patients with cognitive impairment, carers and other health professionals relating to complex health care needs. The CNC’s primary purpose is to improve outcomes for people with cognitive impairment, their families, and carers in the community.The Clinical Nurse Consultant is a clinical expert in Dementia and cognitive impairment who, through leadership, facilitates excellence in evidence based clinical practice, education, research, and professional leadership.The Clinical Nurse Consultant is part of the multidisciplinary Community Assessment and Therapy Team in Prince of Wales Community health. The CNC works with clinical teams to develop, facilitate the implementation of, and evaluation of care management plans for patients diagnosed with or experiencing cognitive impairment related conditions. The role is focused on integrating care and acting as a resource across Community Health, Prince of Wales Hospital and SESLHD.Clinical Nurse Consultant is employed as grade 2 according to the criteria outlined below, and in line with the NSW Public Health System Nurses’ and Midwives’ (State) Award.Clinical Nurse Consultant Grade 2 means : a registered nurse appointed as such to a position approved by the Health Service, who has at least 5 years full time equivalent post registration experience, with at least 3 years full time equivalent experience in the specialty field. In addition the employee must have approved post graduate nursing qualifications relevant to the field in which he/she is appointed to or such other qualifications or experience deemed appropriate by the Health Service. An employer may also require a higher qualification in the specialist nursing field where such qualification is considered essential for the performance of the individual position.Employment of a temporary visa holder may only occur if no suitable permanent resident or citizen of Australia has been identified for this position following suitable labour market testing.All Category A workers and new recruits are required to receive one dose of the seasonal influenza vaccine annually to be considered compliant.Category A workers and new recruits who are non-compliant with seasonal influenza vaccination or have a medical contraindication to influenza vaccinations must comply with all other infection control risk reduction strategies as directed while working in a Category A position.SESLHD is committed to creating a workplace that reflects the diversity of our community. - Current Authority to Practice as a Registered Nurse, Nursing and Midwifery Board of Australia
- Minimum five years full time equivalent post qualification experience, with at least three years full time equivalent experience in an acute/critical care specialty field and post graduate qualifications relevant to the field of practice
- Demonstrated advanced skills in clinical assessment, Community dementia clinical care, and working collaboratively within multidisciplinary settings.
- Demonstrated effective communication and inter-personal skills across a variety of mediums and an understanding of the practice development framework.
- Proven ability to implement changes and develop direction in clinical practice by the utilisation of evidence based research and development of clinical practice guidelines.
- Demonstrated skills in strategic and clinical service planning including an understanding of organisational behaviour, change management and cultural change processes.
- Demonstrated skills in leadership, clinical teaching, and competency in the use of contemporary information systems and experience in the development of specialty education programs.
- Demonstrated clinical expertise in client-centred consultancy with the ability to work independently.
